For calendar-year reporting, Form 5472 is generally due with the pro forma Form 1120 by April 15 of the year following the tax year (e.g. 2025 tax year → April 15, 2026). IRS instructions and Form 7004 govern extensions.
A timely Form 7004 can extend the time to file certain business returns, often moving the extended due date to October 15 for calendar-year filers. An extension extends time to file; it does not necessarily extend time to pay any tax owed.
